The London Borough of Merton ("the Council") is inviting tenders to provide Housing Related Support to residents in the Borough.r r The objective of this contract is to provide Housing Related Support for homeless people from an established supported housing scheme, who are at risk of homelessness and aged between16 - 25 year olds. r r The service will:r r - maximise the ability of service users with mental health and other complex needs, including substance misuse issues to live independently within the community,r r - ensure effective coordination between health and community support agencies in delivering a comprehensive support framework for service users,r r - enable service users to identify and make appropriate health, safety and lifestyle choices to maximise their well-being,r r - support service users to develop their independent living skills to maintain independence in their homes and reduce the risk of breakdown of tenancy and homelessness.
