Nottinghamshire County Council sought a suitably experienced provider / providers to deliver a dedicated Rapid Response and Hospital Discharge Service for Adults in Nottinghamshire (excluding Nottingham City) under a framework agreement.The Service is a short term, home based care service for people who are:— At risk of being unnecessarily admitted to hospital or short term care in a care home due to short term crisis at home, or— Being discharged from hospital and who would benefit from a short period of a reabling or enabling type service to confirm or adjust their on-going package of care.The Provider and any sub-contractors must be appropriately registered with the Care Quality Commission (CQC).Using a competitive dialogue process, the Council identified a Provider to deliver a county-wide service.The framework agreement will run for an initial period of 2 years. There is an option to extend for up to a further 2 years.
