This is a voluntary contract notice for a Part B Service.Delivery of the Health-checks programme in Essex using a Prime Provider model.This includes the NHS Health-checks which is a mandatory public health service for the local authority to provide and Senior Health Checks programme which is particular to Essex.The lead provider will be delivering the programme across Essex.The list below shows the geographical areas covered by this spec. and the relationship to the respective Clinical Commissioning Groups (CCGs):— Locality CCG;— Basildon and Brentwood Basildon & Brentwood CCG;— Castle Point and Rochford Castle Point and Rochford CCG;— Epping Forest, Harlow and Uttlesford West Essex CCG;— Braintree, Maldon and Chelmsford Mid Essex CCG;— Colchester and Tendring North East Essex CCG.The provider will subcontract with GP practices and pharmacies and provide an outreach service in order to achieve set targets. The provider will be responsible for ensuring the selection and invitation of the eligible population including managing all data flows, ensure a call/recall system is in place and be responsible for the verification of all Health Check data. The provider will support and train staff in primary care to ensure the highest quality standards are adhered to, as well as ensuring the highest possible standards and training for their own outreach staff.
