Young people are facing multiple problems that exacerbate each other, and the lack of effective support from services can lead to a downward spiral of mental ill health, drug and alcohol problems, crime and homelessness.The service is underpinned by the vision that people experiencing multiple needs are supported by effective,coordinated services and are empowered to tackle their problems and reach their full potential. The service model will deliver an outcome focused ‘support pathway’ clearly focused on the unique needs of younger service users, through which they will progress, working towards independent living.This is a person centered outcome focused service. It will focus on young people’s individual needs, goals and aspirations, that will enable them to successfully sustain their tenancies, avoid eviction and prevent the risk homelessness.
