The Framework for the Provision of Domiciliary Care Services will be split into two Lots as follows:— Lot 1 — Domiciliary Care for Continuing Healthcare Patients; and— Lot 2 — Complex Continuing Healthcare Packages at Home Service.The Services will support adults (aged 18 and over) assessed as eligible for Continuing Healthcare (CHC) and registered with a Leeds GP, or resident within the boundaries of the 3 Leeds Clinical Commissioning Groups.
