Descriptions
The Framework for the Provision of Domiciliary Care Services will be split into 2 Lots as follows:— Lot 1: Domiciliary Care for Continuing Healthcare Patients; and— Lot 2: Complex Continuing Healthcare Packages at Home Service.The Services will support adults (aged 18 and over) assessed as eligible for Continuing Healthcare (CHC) and registered with a Leeds GP, or resident within the boundaries of the 3 Leeds Clinical Commissioning Groups.Please note that as of 1.4.2018, the 3 Leeds Clinical Commissioning Groups have merged and are now collectively NHS Leeds Clinical Commissioning Group.
