Adult Family Group Conferencing – Providing a family intervention service for older people in Swanse

Funding has been secured to run a 12 month pilot service aimed at older people in Swansea as a response to the ethos of the Social Services and Wellbeing Act 2014. It places itself firmly as an early intervention and prevention service at community/primary care level, focussing on older people who have been identified by health professionals as being at risk of losing their independence, e.g. early stages of dementia, having had a fall at home etc. and providing information at the point of need and enabling families to arrive at their family plan.
