Advocacy Service: Supporting elder care
Descriptions
To collaborate with voluntary sector organisation to support older patients in the Emergency Department, care of the elderly wards and the discharge lounge. To continue with, and develop, a pilot scheme that has been running in the Trust for approximately 6 months to support older patients by supporting discharge home, prevent readmissions and offer an improved patient experience whilst in hospital in order to prevent some deconditioning caused by lack of stimulation.
