Alcohol Misuse Housing Related Support (OFA) Project:The project should consist of a minimum of 8 and a maximum of 14 units of floating support. It is expected that service users should be on the project for up to 6 months as appropriate to service user needs.The support provider(s) will deliver a minimum of 52.5 service hours per week. Staff will deliver support activities in service users’ own accommodation and the community. Staff will additionally provide a 24 hour on-call service.This is a floating support project for vulnerable single people and couples over 25 with no dependent children, who are resident in Denbighshire and who have alcohol related support needs. All service users will be homeless or at risk of becoming homeless and will have been identified as needing support in a number of areas to enable them to manage their tenancy independently. Support will be time limited and it is expected that service users will move on to live independently in the community or onto a community based project with lower level support.Referrals will be accepted from a range of agencies to be specified / agreed by the Supporting People Coordinator. The provider will develop referral routes to encourage access to individuals not known to services i.e. district nurses, GPs etc.Project Aims include:Preventing people from presenting as homeless or potentially homeless.Preventing vulnerable people being placed in unsuitable accommodation.Ensuring vulnerable people can move on to more independent living and continuing to live independently after support has been withdrawn therefore breaking the cycle of repeat homelessness.Ensuring vulnerable people are supported to access education, employment, training or voluntary work and have the opportunity to achieve their full potential.Increasing or maintaining individuals’ engagement with treatment or re-engagement with treatment.Promoting a more stable lifestyle and reduction in alcohol misuse for those that do not wish to access treatmentContributing to relapse prevention Contributing to decreased social isolation Contributing to improving social inclusion & Community integration
