Somerset Partnership NHS Foundation Trust is currently in the process of constructing a new Community Hospital for the population of Bridgwater. Somerset Clinical Commissioning Group (CCG) is seeking to appoint a suitably qualified and experienced healthcare service provider for the delivery of an innovative service that will broaden and drive improvements in the delivery of ACSC and Traditional Ambulatory Care Services within the Bridgwater Community hospital.Somerset CCG are looking for a provider that will not only deliver traditional ambulatory care services but will also deliver a holistic and a whole system approach for patients by exploring a rural take on the Kings Fund report 'Emergency Hospital Admissions for Ambulatory Care-Sensitive Conditions: Identifying the Potential for Reductions':— reactively selecting patient groups that have already presented at A&E, acute outpatients or inpatients and working to meet their needs in the community hospital setting— proactively selecting patient groups that are at increased risk of becoming inpatients at acute hospitals, A&E or in need of frequent outpatient appointments.Potential providers should note that Somerset County has a dispersed, rural population and therefore some elements of the report may not be applicable to this area.Somerset CCG defines Traditional Ambulatory Care as a personal health care consultation, treatment or intervention using medical technology or clinical procedures delivered on an outpatient basis (i.e. where the patients stay at the hospital or clinic, from the time of registration to discharge, occurs on the same calendar day)Traditional Ambulatory Care covers:— Blood transfusions, where medical assessment and review is not required.— Management of PICC Lines— Intra venous infusion for osteoporosis treatment— Intravenous therapies— Venesection— Urological procedures including:- change or removal of urethral catheters- TWOC (trial without catheter)- Measure urinary flow rates and checks of residual flow- Change of supra pubic catheterThe potential provider will be required to work closely with the Somerset Urgent and Emergency Care Steering Group in order to identify and transfer patients from the acute setting.There will be a clear link between ambulatory care services and the Frail Elderly Pathway and long-term conditions. The ambulatory care pathway will not exclude palliative care patients.The potential providers will be asked to provide a population/patient-based approach and to work with patients in order to move them towards a self-management approach where appropriate; inevitably this will require enhanced secondary care clinical input. Somerset CCG envisages the service will have two approaches – rapid access and planned procedures, therefore this will be a medically led model rather than a nurse led model.To achieve the aims of the service, any potential service provider will need to demonstrate how they intend to create a holistic model of care that takes into consideration mental health, transport and social care needs. In addition the strands of the Equality and Diversity model will need to be catered for.The successful bidder will perform the services from the new Bridgwater Community Hospital and will lease clinic space from Somerset Partnership NHS Foundation Trust who will own the new community Hospital facilities.The aim of the procurement is to acquire sustainable, high quality, value for money provision of ASCS and traditional ambulatory care for the community. The service will:— support local waiting times, targets and activity plans— provide patient choice and diversity— provide assurance to the commissioner on performance and service delivery— have patient-focused outcomes, that are derived by individual patientsThe volume of activity for the successful applicant will be dependent on the service design, provider ambition and patient choice.The potential providers will be asked to provide a population / patient-based approach to the services.The volume of activity for the successful applicant will be dependent on the service design, provider ambition and patient choice. Somerset CCGs published Commissioning Plan includes a key ambition to ‘help people recover from episodes of ill health and injury' and as part of that intend ‘to develop and implement ambulatory care pathways' across the countyIt is the commissioner's intention to work closely with bidders through a two stage Invitation to Negotiate process to produce an innovative ACSC and traditional Ambulatory Care service specification in line with Somerset CCG strategy.
