App - Personalised Care Plans support choice for pregnant women

Descriptions

Pregnant women will have more influence over the care and support they receive thanks to the introduction of the Personalised Care Plan (PCP) to Northamptonshire. PCPs are completed and owned by women individually, and enables them to set out their values, priorities, expectations and concerns about their maternity journeys before, during and after they give birth. The Northamptonshire PCP was launched in August 2019, link below: https://www.northamptonshirehcp.co.uk/2019/09/https-www-northamptonshirehcp-co-uk-2019-09-personalised-care-pregnant-women/ The Northamptonshire's PCP template has been developed through engagement with maternity service users as well as midwives, health visitors, obstetricians and local maternity care leaders. •The PCPs will provide a tool for women to document and share what is important to them about the care they receive and the decisions they make. •The PCP covers 'About me', 'health and wellbeing in pregnancy', 'personalised birth preferences', 'after your baby is born', 'birth reflections'. This can be completed electronically or printed. •They also give women opportunities to identify, at different stages of their pregnancy, if they need more information in order to make choices. •Bengali, Romanian, Lithuanian and Polish-language versions are available and electronic versions of PCPs are set to be developed as an option for women if they prefer. Northamptonshire CCGs are looking to engage with suppliers who can provide/develop an Application (app) with the features specified in the attached Expression of Interest (EOI) Form, including, but not limited to; •Provide a digital version of the Northamptonshire PCP, which was launched in August 2019. •Provide Information about the local maternity units. •Appointments which feed into the calendar for Apple iOS, Android OS and other operating systems. •Provide detailed evidence based information (as agreed and audited by Maternity staff/obstetricians/service users) on pregnancy, (including health and wellbeing, antenatal care, getting ready for birth and beyond) •Provide labour and birth, (including early signs of labour, what to expect in labour and birth, assisted birth, immediately after giving birth) •Provide Postnatal care (including postnatal care explained/breastfeeding/mental health/physical health and wellbeing, baby practicalities, caring for your baby and compliant with Baby Friendly Initiative) •Allow users to provide feedback to NHS/PALS and the App supplier •Proven track record in successfully delivering NHS apps to other NHS organisations •Delivered prior to March 2020.
