Tenders are invited for the delivery of the Aros Byw Bod Adref project.This is a time-limited, medium intensity floating support project for people who are experiencing early signs of dementia, as well as people who could potentially experience difficulties in maintaining their independence due to memory problems, depression, psychosis, or similar issues.Individuals supported will be resident in Denbighshire, either homeless or at risk of becoming homeless, and requiring support to enable them to manage their accommodation independently. Support will be time limited at a maximum of 2 years. The project will support a minimum of 10 people at any one time.This is a 3 year contract, with the OTE for 2 years (the total contract value given within this notice is for the period of 3 years).Note: DCC is using Proactis to carry out this procurement process.
