The service will:— prevent unnecessary hospital admissions,— support and resettle people following a hospital admission,— encourage people to regain their confidence and capabilities,— refer onto other community support as required, e.g. neighbourhood teams for assessment of long term needs.‘activities can include: support with personal care, medication support (including collection), bathing, food preparation and collection of light shopping,repositioning of a person and ensure a comfortable living environment’.
