Care and Support at Home Tier 2

Descriptions

The service is to provide domiciliary care predominantly for older persons in Hampshire;The Tier 2 call-off will be looking for providers to deliver domiciliary care across Hampshire. The County will be split into 15 zones with providers able to tender for more than 1 zone. Providers can have a tier 2 contract in multiple zones.Where a tier 1 provider in the zone can not accept a package of care it will be referred to the tier 2 providers. Tier 2 providers who are able to start the referral by the set date will compete on price with the lowest priced provider being awarded the package of care. There is no requirement for a tier 2 provider to bid for any of the packages of care on tier 2.The service delivery model is made up of 2 elements, the 12 week post assessment period and long term care. In the 12 week post assessment period the providers are expected to take a strength based approach to delivering care to look to right size the package.
