The aforementioned contracting authorities are working jointly as a Transforming Care Partnership (TCP) to deliver a programme to commission a small number of providers to work in partnership with us to develop and deliver bespoke and personalised care and support for individuals aged 16 years and over stepping down from secure services into the community. The TCP may also require providers to work with individuals at risk of admission.We are seeking high quality providers with proven expertise and experience of delivering personalised packages of support for people stepping down from secure services into the community and must have experience and knowledge in supporting individuals with learning disabilities and/or autistic spectrum conditions and in supporting individuals with forensic needs as appropriate.
