Descriptions

The provision of a community prevention service for adults living in Buckinghamshire to reduce, delay and/or prevent the need for social care support or services. The service will identify needs early, provide timely information and advice, prevent crises, and promote independence, focusing on individual support, digital solutions, and collaboration with the Voluntary, Community and Social Enterprise sector.
