The Tri-borough Steady & Stable Falls Prevention service is an evidence based falls awareness and prevention service to; improve balance, reduce falls and keep people living independently. The service outcomes are ; •Improved functioning e.g. posture, balance, mobility, orientation and safety awareness •Prevention/reduction of unnecessary hospital admissions and other acute care as a result of a fall •Increased proportion of older people supported in their own homes •Enhanced choice and control over their lives
