To provide community based support for twelve adults in Henllan, Denbighshire. The contract will run initially for a period of one year renewable for a further 5 years subject to satisfactory performance and annual reviews.Denbighshire has been working with a housing association to develop 3 bungalows. The bungalows will provide rented accommodation for individuals with a range of needs, including adults with severe learning disabilities, people with high functioning autism and those with physical or sensory impairments.In order to support the individuals a tenancy based support service will need to be established. As each bungalow will support individuals with quite distinct needs, the support service will have three elements. these three elements of direct support will be provided by one Provider agency.
