Descriptions
There is a need to procure to the role of Third Sector Broker as part of the wider Intermediate Care Services Model. The role will be required to work across the Adult Services Common Access Point, integrated Hubs and Hospital Teams. The role will require the means to provide up to date information on the Third Sector Provision, be able to identify signposting opportunities, broker services in response to identified needs and highlight any gaps in provision for referral.NOTE: The authority is using eTenderwales to carry out this procurement process.
