DCC are seeking to award a contract for the delivery of care reviews and associated administration for Adult Care Management.The key strategic priorities are ensuring eligible needs of the person are appropriately met and identify responsible financial savings to the persons care package.The review process includes but is not limited to:— Considering information on the person's file;— Gathering information from other agencies involved with the person;— Meeting with the person directly and where applicable consulting with their representative;— Re-assessing the persons needs and eligibility where appropriate;— Considering personal and community assets that are available to support;— Evaluating how effective their support has been in achieving the outcomes that are important to the person;— Exploring options for them becoming more independent;— Quality Assuring each completed care review;— Provider engagement / negotiation;— Inputting the quality assured care reviews into CareFirst.
