CTP668 – Derbyshire Integrated Wellbeing Approach.
Descriptions
Derbyshire County Council is commissioning an Integrated Wellbeing Approach which will bring together a number of Public Health lifestyle-related services that are currently provided separately. The integrated service shall offer a better quality service to clients, as well as providing opportunities for increased efficiency and greater value for money.The aim of the approach is to address wider determinants and inequalities in health, to increase resilience in individuals and communities and to provide a range of person-centred services that are delivered according to need which will contribute to people staying well and living independently for longer.Services included as part of the Approach are the Wellbeing Service, Smoking Cessation (Community and Specialist), Weight Management (Community and Specialist), Making Every Contact Count (MECC) as well as inclusion of a website as an integral part of service delivery. It also includes administering and monitoring the Health Referral Scheme which has been commissioned separately, but remains an integral part of the Derbyshire Integrated Wellbeing Approach.The approach will deliver a managed wellbeing service based on a Primary Contractor model with one provider awarded the contract to provide and/or manage the provision of the services.
