The improvement of services for older people is a key priority for both the NHS and High Weald Lewes and Havens CCG as projections show that the local population, which is already older than that of most other areas nationally, is likely to continue to age. Dementia is therefore a key area of concern. The number of older people with dementia in the CCG is projected to increase by 24% by 2020. Improvements in diagnostic provision are evident within the CCG and rates of diagnosis are increasing. The result being that early access to support services, to enable people to live well with the disease, assumes a high priority. The publication of the National Dementia Strategy has intensified our focus on the improvement of services for older people with dementia in High Weald Lewes & Havens and evolving evidence-base and emerging models of best practice, will provide opportunities to reflect on how well local provision is meeting the needs and aspirations of people living with dementia and their family carers. The Dementia Post Diagnosis Support Service will be a key element of the overall dementia care network in the CCG area and its effective, efficient and person centred delivery is a high priority for the CCG. Key to this HWLH Commissioners will require interested parties to be able to evidence a demonstrable ability to work in partnership with stakeholders, in the best interests of patients and carers. The contract will be for two years duration, thereby facilitating the CCG's aspiration to move towards a fully integrated model of dementia care provision in the foreseeable future. The Commissioners reserve the right to extend for a further 12 months.
