The DCC Homelessness Prevention Team are recommissioning housing related support services, with a view to achieving better outcomes and equality of access.Housing related support enables people to establish and/or maintain independent living. Support will be flexible and person-centred, but may include things like budgeting, including accessing money advice; developing life skills; achieving safety and security, and accessing other helpful services and opportunities.Currently having the opportunity to remodel 4 existing separate contracts, we want to create a single service under one contract, with a focus on person-centred, holistic support approaches and outcomes. The service would be flexible and needs-led - available to anyone in Denbighshire who is homeless or threatened with homelessness, and who may also have one or several other support needs (e.g. substance misuse, mental health issues). With a focus on homelessness prevention, the service would complement other specialist services, and reduce duplication. The project would be split over 2 Lots – 1 for all young people’s support and families supported housing, and the other for over 25s. Both Lots will include floating support and supported housing.Project Aims:- People with lived experience are able to play a key role in shaping the delivery and development of the project.- People are able to achieve good quality of life and wellbeing.- People are prevented from becoming homeless or threatened with homelessness.- People are safeguarded and protected from harm.- People are able to live independently, increasing their confidence and independent living skills, and preventing dependency on services.- People are able to manage their housing tenure, including being able to:Pay for their accommodationMeet the responsibilities of having a tenancy or owning their own homeKnow how to get all necessary utility services at their accommodationDeal with repairs and/or improvements to their propertyKeep warm, comfortable, safe and secureGet on with their neighbours- People are able to benefit from formal and informal support networks when needed.- People do not experience social isolation.- People are supported to achieve their full potential, including accessing education, employment, training, and voluntary work.- Where needed, people are able to resettle into suitable accommodation – working with social and private sector landlords to source accommodation for a range of needs.- People are able to move on from support when they are ready.- Partnership working to deliver joined up outcomes and develop services, to contribute to wider needs - with innovation in this working, e.g. considering co-location, shared support plans, and shared resources.Supporting People Programme Outcomes:- Personal and community safetyPeople will be supported to:feel safecontribute to the safety and wellbeing of themselves and others- Independence and controlPeople will be supported to:manage accommodationmanage relationshipsfeel part of the community- Economic progress and financial controlPeople will be supported to:manage moneyengage in education/learningengage in employment / voluntary work- Health and wellbeing.People will be supported to:be physically healthybe mentally healthylead a healthy and active lifestylePlease note that this proposed remodel is currently out for consultation (until 12/09/17), therefore elements of the project are subject to change following feedback received during the consultation period.We are holding a 'Meet the Buyer’ Event 13/10/17, 1.30-3.30pm in the Council Chambers of Russell House, Churton Road, Rhyl LL18 3DP.
