The Local Auhtority wishes to appoint a suitably experienced Service Provider who is registered under RISCA to deliver domiciliary care services. Domiciliary care and support is required for a young person of 15 years of age with Cerebral Palsy living in her own home with her parent in the Llandudno area. The support required is detailed below in general and with future potential to increase over time:•1 hour a day Mon- Friday to assist with showering and preparation for school. 7.30- 8.30am•4 hours every other Saturday with showevering and taking her out to do activities. These 4 hours can also be used in the school holidays.•3 over nights- sleep ins per weekWe are looking for one Service Provider who would have capacity and experience to provide person centred care and support to assist the young person remaining at home in the community with her parent.
