The domiciliary support service will include home care services to Adults who have a Physical Disability or Frailty. These services are offered on the basis of the overarching principles identified in the Social Care and Community Plans of the Council, that all service users should enjoy their basic human rights by being treated in a way, which enables them to:-Retain their individuality-Maintain personal dignity-Exercise self-determination-Live a normal life within the communityThe objectives and outcomes of the service to each user will be identified within the service user’s Care Plan and the specific tasks to be performed to achieve these will be set out in the service users Provider Plan. The Provider Plan must identify the context within which these specific tasks will be undertaken and must respond directly to the outcomes identified within the Care Plan and assessment of need. They may involve one or more of the following:-Assisting/supporting the service user.-Assisting/supporting families and carers.-Direct care for the service user.-Support for the recovery or rehabilitation of the service user.To include the following·Personal Care; Domestic Services; Services to informal carers; Night service; 24 hour care packages. Care services are required 24 hours a day, 7 days a week, 365 days a year including weekends and Bank Holidays. The indicative number of hours proposed to be contracted across the County Borough, for the Domiciliary Support Service for adults with a Physical Disability or Frailty within the areas of Rhondda Cynon Taff County Borough Council will be 3400 per week.It is anticipated these hours will be split into 3 lots. The details of the split will be made available at tender stage and organisations are welcome to apply for 1, 2 or all 3 of these lots.
