Advance notice of procurement of Discharge To Assess (D2A) service. Initial 6-month pilot, followed by benefits assessment, and potential longer-term on-going service. Caseload up to 60 normal patients, plus 9 non-weight-bearing. Covers discharge from three acute sites: KCH (Canterbury), QEQM (Margate), WHH (Ashford) . Budget for pilot is £1.1M. D2A ‘Home First’ - model provides an opportunity for patients to be transferred form an acute hospital bed in a timely way to an appropriate alternative community setting for on-going assessment and rehabilitation. The model is focused on a more proactive management of frail older people and improving their long term outcomes. The commissioner is NHS Ashford CCG as Lead Commissioner and NHS Canterbury and Coastal CCG, NHS South Kent Coast CCG and NHS Thanet CCG as Associate Commissioners. Timeline 10 Aug 2015 - Publish tender documents 19 Aug 2015 - Conduct bidder information event (venue TBA) 09 Sep 2015 - Receive bids 18 Sep 2015 - Notify bidders of outcome 29 Sep 2015 - Sign contract 19 Oct 2015 - Service live Further details - memorandum of information and service specification - will be in tender documents.
