The increasing demand for health and social care alongside the financial and workforce challenges in the sector are unprecedented. In addition, the impact of the Covid 19 pandemic means that the systems and people who deliver health and social care (staff, unpaid Carers and volunteers) are stretched beyond comfortable limits. In directing the partnership resources and efforts towards a clear purpose of ‘enabling people to live well at home for longer’ the health and social care partnership is seeking a programme to support the cultural and organisation changes required to help build the capacity and capability of the system to meet the inevitable winter pressures. Working in partnership to design and deliver a programme that will support the attainment of ‘enabling people to live well at home for longer’ while developing an organisational culture (further information included in specification).
