Provision of integrated primary care services from 1st April 2018 for 12 months with the possibility of a further 12-month extension. Integrated services include the following:— ambulatory blood pressure monitoring,— anti-coagulation monitoring and initiation,— case finding,— care planning & case monitoring,— complex common mental health,— coordinate my care, diabetes (high risk) and level 1 and level 2,— ECG,— homeless care,— near patient monitoring,— ring pessary,— severe mental health (level 1 and level 2),— severe mental health shared care prescribing,— spirometry testing,— wound care,— carers,— end of life care,— A&E frequent attendees,— asthma,— extended hours, ad,— locality plan.
