The provider shall ensure patients shall be referred to the service by their GP, and other health care professionals. The provider shall ensure that once the patient receives a referral from their GP/other agreed healthcare professions, a qualified exercise specialist shall design a tailored exercise programme, for the patient to complete over twelve (12) sessions.The provider shall ensure patients experience the benefits of a physically active lifestyle and shall be encouraged to continue physical activity to maintain this change over time.
