Flintshire County Council would like to appoint a trusted service provider to develop and deliver a new Community Wellbeing Service. This new service, funded by Welsh Government Regional Integration Funding (RIF) demonstrates a commitment to provide preventative support for adults within their own home or close to home as an alternative to hospital admission.The commissioned provider will establish a small team of Community Wellbeing officers across the community in Flintshire. The service will adopt a preventative approach to provide practical support, predominantly to older people, and those identified as being at higher risk of escalating needs. This will include support for people recently discharged from hospital to avoid re-admission or to maintain their wellbeing at home. The Community Wellbeing Service will provide supplementary capacity in support of the Single Point of Access (SPoA) service model and emerging health and social care Multi-professional teams in Flintshire.
