the provision of commissioned care to children with continuing care needs aged 0-18 year’s old living in the Hywel Dda health community.The Contractor must demonstrate its commitment to providing a model of holistic care for CYP for whom care packages have been commissioned.This service is for CYP usually resident within the counties of Carmarthenshire, Ceredigion and Pembrokeshire who have identified Continuing Care needs for whom a bespoke package of care has been commissioned through the multi-agency Complex Needs Panels and funding approved by Hywel Dda University Local Health Board and/or the relevant Local Authority in the case of jointly/tripartite funded packages of care. When contractors respond to the pen portrait they will need to identify the number of active carers to deliver the care package. Active Carers – carers that can be committed to deliver the care package on a minimum of a fortnightly basis by the Contractor. This should include cover during times of absences such as sick leave/annual leave. This will ensure continuity of care and that knowledge and skills are maintained. Funding to train these identified number of carers will be supported in the initial period by the Authority. However, funding for additional carers will be the responsibility of the Contractor i.e. new carers being introduced to deliver the care package due to the initial carers leaving/being withdrawn from the care package.The Contractor will be expected to work towards developing organisational capacity to deliver services across all three counties of the Hywel Dda health community.
