Framework for the Provision of Step Up/Step Down Beds

The service to be provided is defined as:“Short stay Step up : down beds to re-able people so that they can return to their own homes without the need for hospital admission or with a reduced stay in hospital.The service will be provided in specially-selected independent care homes for a period of up to 6 weeks. This will be achieved through the use of therapeutic interventions identified in a ‘treatment plan’. Interventions will be provided by trained care home staff with input from the Intermediate Care team.”This project will help us make sure that people in Wrexham receive the right care, at the right time, in the right place, and by the right professionals.This service will be jointly commissioned by Betsi Cadwaladr University Health Board and Wrexham County Borough Council.12 beds have already been block-purchased in care homes; this advertisement refers to the provision of the same service but through a spot purchase arrangement. This means you will not be expected to hold the beds for us to access and, as such, there is no guarantee of work from us.Having pre-approved providers able to take placements on a spot-purchase basis will provide commissioners with options when block-purchased beds are full.
