HCC — 10/15 — Provision of Community Services to Support People with Dementia and Memory Problems.

The Council wants to commission a countywide Service that provides a range of opportunities (skills maintenance activities, social activities, emotional support and practical advice) to help people with dementia and memory problems live well and support their carers/family.The requirements have been developed to help address the priorities and objectives set out in Hertfordshire's new Dementia Strategy 2015-2019 and known issues with current services, including:— Making community activities/support more accessible to under-represented groups.— More flexible support, tailored and promoted on a primary care locality basis.— Focusing on the needs of individuals (including better prioritisation).— Better use of community capacity and local demographic data by providers.— Extra capacity to meet increasing need due to increase in prevalence and diagnosis.It is anticipated that significant partnership and collaborative working will be required to deliver this Service successfully to all areas of Hertfordshire.The annual budget for this Service is 516 000 GBPThe Contract will be for an initial term of 2 years, with the option to extend for a further period of up to 24 months.Full Details in relation to this procurement are in the the ‘My Tenders’ area in In-Tend.This procuremnt is an Open (1 stage process).
