Public Health Commissioning in Trafford are looking to commission a single service or a number of services to provide direct support to reduce health inequalities in priority groups. This will include smoking cessation, physical activity, health screening, mental wellbeing, substance misuse and nutrition. r r The project will target those groups facing health inequalities:r r -People living in high areas of deprivation as defined by the 20% most deprived nationally.r -Ethnically diverse populations experiencing health inequalities.r -Older people from deprived communities.r -People with learning disabilities, physical disabilities or sensory impairments.r -Trafford residents in contact with the criminal justice system. r -Travellers/gypsy communities in Trafford.r -People who are LGBTQIA+.r Services will be commissioned that are able to deliver against the following outcomes:r r •Increase physical activityr •Reduce numbers who smoker •Reduce harms from drugs and alcoholr •More people screened for cancer and other conditions e.g. health checks/dementia/diabetesr •More people at a healthy weightr •Improved mental wellbeingr •Improved wider determinants of health (reduce poverty, increase employment and volunteering, better environment, improved housing)r •Reduce inequalities in health, especially impacting those with the poorest health outcomesr •People less likely to die from causes that could have been preventedr r Please note that the procurement is being undertaken under the Light Touch Regime.r
