a) The Hospital Discharge element of the service involves delivery of short-term(normally up to 14 days) support and care for people who are being dischargedfrom hospital.b) The Community element of the service is for people living at home whoexperience a crisis. It aims to avoid unnecessary hospital admissions oremergency short-term admissions to a care home. People will be able to accesshis service for a limited time (normally up to 14 days), until they regain sufficientindependence or until longer term arrangements can be put in place or untilheir Carer/s can resume their usual caring role.
