• Reduce the number of re-admissions within 28 days for residents who have accessed the Home from Hospital Service (excluding those with scheduled appointments).
• Ensure supported discharge from emergency departments and medical assessment wards to avoid unnecessary admissions.
• Increase the number of people who are able to remain sustained in their communities following a hospital admission.
