Home Parenteral Nutrition.

Descriptions

This contract is for the provision of an All Wales Home Parenteral Nutrition Service for adult and paediatric patients who are resident or registered with a GP practice within Wales. It should be noted that there will be by exception a small number of patients who are under the care of Consultants in Wales but are residents in England. The HPN Contractor will be responsible for providing the HPN feed/fluid and associated equipment including delivery and 24 hour on call advice as appropriate.Specialist homecare nursing may also be required.This covers the following therapies and medicines.— Home Parenteral Nutrition.— Subcutaneous Magnesium Therapy.— Bisphosphonate Therapy.— A Short course of Intravenous Antibiotics.— Temporary Suspension (On Hold).There are currently 81 patients undergoing home parenteral nutrition therapy and an additional 5 on subcutaneous magnesium. It is now estimated that the number of patients requiring HPN will increase to around 100-110 during the contract period. On average HPN patients will require 5 PN feeds each week. The workload would be approximately 26-28,000 PN bags manufactured each year.There are also a small number of patients not covered by the policy receiving HPN for palliation on a short term basis usually with nursing support. Funded on an individual basis through their Health Board.
