Housing services. The service will provide housing related support to older people in the city over the age of 55. Floating support delivered will help to facilitate hospital discharge, prevent re-admission to hospital and enable people to maximise their level of independence and well-being for as long as possible in their own homes in the community. The Supporting People service will help people to achieve aspirations around; living well, ageing well, staying well and working well.
