Wakefield Council has developed a new model of provision for Housing Related Support, designed around:* Identifying the right services for people the first time;* Delivering long term solutions to a person's homelessness, with as few transitions as possible;* A Making Every Adult Matter (MEAM) approach — offering coordinated interventions that respond flexibly to meet assessed need;* A focus on harm reduction, recovery, resilience and independence; and* Promoting positive approaches to health and wellbeing.The model is comprised of 3 integrated pathways (detailed below) accessed through a single point of access:* Single Point of Access;* Assessment Pathway;* Access to Housing Pathway:— Main Provision,— Specialist Provision (this not included in scope of this procurement exercise);* Housing Sustainment Pathway.
