The National Clinical Audit of Care at the End of Life requires collection, analysis and effective report of robust comparative data on the quality of care and patient outcomes for patients in the last few days and hours of life, being cared for the NHS funded services in England and Wales. Engagement with clinicians, patient groups, commissioners (both local and national), service providers and major charities is essential.The overarching aims are:a) A case note audit of in-patients in hospitals in the last few days and hours of life;b) Organisation Audit;c) Carer Reported Experience Measures;d) Staff Reported Measures;e) Periodic, time-limited, topic-specific ‘spotlight’ audits;f) Feasibility study for roll out to community settings.
