The principal aim of this contract is to support an independent living strategy for disabled and disadvantaged residents in Anglesey. The aim of the service is to act as a conduit for individuals who are isolated within their disability to move forward to integrate with the wider community.The contractor will provide the individual with an initial confidence building program with onward development of basic skills and certificated educational activities. In the context of this contract the term basic skills will encompass skills required for an individual to live a fulfilling and independent life. These skills will include socialisation, financial management etc. as well as basic numeracy and literacy skills.A requirement of the contract is that the environment created will be socially inclusive, safe, and inspiring. However, the contractor must strive to avoid creating a traditional ‘day service’ ethos, where individuals are not provided with support to progress and achieve their own maximum potential and aspirations. The contractor is therefore required to clearly evidence time limited progression plans for each individual, moving them through a set program of activities which are modular, documented, and fit under one or more of the Outcomes outlined below:Outcome 1:Service users are more confident, happy and fulfilledOutcome 2: Service users are more independent, and have improved levels of basic skillsOutcome 3:Service users are fully involved in the planning and delivery of their progression plans. Outcome 4:Service users are referred onto new modules or services at the right time with the right transitional support, demonstrating progression and personal development to achieve their aspirations and potential.The contractor will provide evidence on a quarterly basis for achievement against these Outcomes e.g. for Outcome 1, measures will need to be developed in order to ascertain if service users are more confident, happy or fulfilled following completion of a module (such as the use of pre and post questionnaires); for Outcome 3, evidence will be required as to the level of engagement from service users in the planning and delivery of their progression plans. As an addition to the ethos of social integration it is expected that participants will be encouraged to move on to other services and activities not provided by the contractor, as outlined in Outcome 4 above .The modules and services provided must integrate with the aims and objectives of the Taith i Waith program.Each module provided will have a set time scale for each individual; the average time for completion of each module should not exceed three months. However, it is accepted that the actual time for module completion will vary for each individual. The contractor must document the time taken for each individual and ensure that strategies exist to attain the average three month target.
