This is a 1 year contract (with a possible 1 year extension option) to provide Torbay and Southern Devon Health and Care NHS Trust with an outcomes based Independent Living Enabling and Support service for Adults in Torbay to support Service Users to remain living as independantly as possible within their chosen community. These Service Users have a wide range of conditions which may include learning disabilities, poor mental health, cognitive impairment, autistic spectrum, offenders, individuals with drug and alcohol issues, post traumatic stress and those with communication and sensory needs. The service may work with single people and couples. The Provider should have a range of housing options to offer the Service user or an outreach service to support them find suitable accommodation.
