Integrated Musculoskeletal Service for Telford & Wrekin CCG.

Descriptions

Telford & Wrekin Clinical Commissioning Group (CCG) currently commission health services for approximately 170,000 residents and are Inviting Expressions of Interest from potential Prime Contractors to deliver a Community Integrated Musculoskeletal (MSK) Service to be based at a minimum of two location sites within the Telford & Wrekin CCG area. This service shall serve as an alternative to hospital based treatment for the majority of patients being referred by GPs for MSK conditions. The proposed Community Integrated MSK Service shall provide a single point of access with triage for referrals to MSK assessments, Physiotherapy, Rheumatology, podiatry and the management of chronic pain.A Prime Contractor is being sought for these services, who shall be responsible for the management and delivery of the entire care pathway and may enter into sub-contracts with other Providers for part/s of the care pathway. The successful Prime Contractor must fully meet the service specification and demonstrate innovation, best quality, improved patient experience, cost effectiveness, provide excellent access and deliver seamless and clinically safe patient pathways. There is an expectation that this service shall achieve savings against the existing service model (currently based on National Tariff). The successful Provider shall be required to sign the relevant NHS Standard Contract for these services, accepting all contract terms and conditions.All other relevant information shall be contained within the Service Specification.
