NHS West Norfolk Clinical Commissioning Group requires to integrate the current provision of adult palliative care services for patients registered with GP surgeries in the Commissioner’s area in West Norfolk. The West Norfolk Integrated Palliative Care Service (IPCS) will work as an integrated part of health and social care provision throughout the West Norfolk locality. The integrated services will effectively manage, co-ordinate and provide palliative and end of life care resources that will deliver improved patient outcomes, equity of access to services, service efficiencies, and better value for money.
