SummaryNWSSIC and the Health Board are working collaboratively across North Wales to establish, strengthen and improve joint working between health and social services within the community.This commission will be funded from Welsh Government grant funding which has been identified to support the implementation of the Social Services and Well-Being (Wales) Bill.Outline briefExpression of interest and quotation is sought to work with the 6 Local Authorities and the Health Board in North Wales to undertake the following:-Ensure that work currently being developed around continuum of community services across Health and Social Services is completed and adopted across both social care and health services.-Work with both the Health Board and the Local Authorities across North Wales to ensure that the joint requirements in the ‘Framework for Delivering Integrated Health & Social Care – For Older People with Complex Needs’ are delivered across North Wales.-Work with the Health Board and Local Authorities to develop and agree a governance and accountability framework for locality working.-Ensure that the outcome and areas of development identified t-e following recent partnership review of locality teams working across North Wales are considered and actioned where necessary to enable effective multi-agency locality team working.-Work across Local Authorities and the Health Board to review current interface practices and care pathways within community services and identify areas of good practice ensuring that this is replicated across all localities.-Identify areas where change is required to enable locality working to develop to its maximum potential and agree an action plan across health and social services to enable this to happen.
