Integrated Social Prescribing Service for Royal Borough of Windsor and Maidenhead

NHS Frimley ICB is seeking expression of interest in a potential tender for an integrated social prescribing service in the Royal Borough of Windsor and Maidenhead Place. The service will be open to all residents of the Borough. This social prescribing integrated service is for all residents to access that could potentially benefit anyone who would like support to maintain, manage or improve their health and wellbeing. Integration with existing local partner organisations is essential for this service, specifically working in partnership with our Primary Care Networks and community/voluntary partners The main objectives of the service are: •Provide wide access to a social prescribing service and associated support offers for all residents who would benefit from a social prescribing intervention •Proactively reach into and have visibility across the community and key locations of local partner organisations. •Close collaboration and engagement with voluntary, community, faith and social enterprise organisations and community groups and charities, operating in and /or working with residents in the borough. •Identify through evidence gaps in local community resources, in order to collaborate in further development of existing resources and participate in the implementation of solutions, with particular focus on inequalities. •Improve resilience and social support for individuals through effective signposting, provision of information, advice, and support, and connecting individuals to community groups and activities to reduce dependency on and inappropriate use of health, social care and other services. •Create mechanisms to increase the number of contacts received from residents and local partner organisations to contribute to enhancing the health and wellbeing of individuals accessing the service for non-clinical support and to increase self-care. •SPLWs to actively work with individuals to personalise every contact, improve social connections and support the borough's efforts to reduce social isolation and loneliness, amongst other things. •The flexibility is a key requirement especially at the early stages of introducing this service as also the willingness to pro-actively seek opportunities to develop and improve the service. The contract will potentially be for 3 years with an option to extend for 2 further years. The estimated contract value based on current activity levels would be £900k over the full five-year period. This process is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
