The contracting authority is looking to develop an IT solution to support the management of patients across multiple healthcare organisations. The initial implementation will support those patients designated as requiring specialist care in the last 12 months of life. The system should be flexible enough to enable the contracting authority to add at a later date patients suffering from defined long term conditions, e.g. dementia, diabetes, etc.The information system will allow interrogation of patient data by all clinicians and care providers relevant to the respective patient and will also provide a facility for patients to manage their care needs by presenting data via a relevant patient portal. The system will have a data warehouse at its core holding information updated from relevant source systems managed by organisations relevant to the care of the given patient group. These interfaces will allow for two-way interoperability.
