Long Term Medical Conditions Educational Support to enable those diagnosed to self manage their conditions. There will be 2 distinct educational programmes. Generic sessions and disease specific sessions which will be delivered and repeated on a monthly basis for a total of 6 times. The contractor will design, develop, deliver and evaluate the programmes to be determined once the tender has been awarded. The sessions will be for a duration of 2 hours and held in a non-clinical venue that will accommodate a capacity of up to 200 people with a minimum number of 50 people per session.
