The purpose of this longitudinal study is to identify and evaluate the social and economic costs of re-providing residential care and support in two new extra care settings in Bridgend.It is a unique opportunity to case track the assumptions and decision making that supported the commissioning of these two new schemes and map it against the real outcomes for people, their families and the staff involved.Over a period of 10 years this study will collect data on the experiences of all involved. This primarily be the level of care and support required before moving into extra care and in the following years, quality of life and financial measures. This will include information on how many people were able to live out their lives in extra care without moving to a registered residential/nursing care setting. It will also capture medical data such as the use of primary care and district nursing services, number of falls requiring the ambulance service or hospital admission, health service use, admissions to hospital and where the individual went following hospital if they could not return to their own home.The review must also include emergency admissions to hospital, delayed transfer of care, levels of care packages, appropriateness of accommodation, use of GP service and consultations as well as resident, tenant and family satisfaction levels, environmental considerations, value for money and equality across service user groups.The study will also track referral routes, length of stay, demand, void levels and changes to service model.Information may be gained through focus groups, surveys, consultations with internal and external stakeholders.NOTE: To register your interest in this notice and obtain any additional information please visit the Sell2Wales Web Site at http://www.sell2wales.gov.wales/Search/Search_Switch.aspx?ID=50391.
