The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of Cardiff and Vale University Local Health Board who are seeking to establish a contract for the provision of a Low Level Community Based Support to Facilitate Admission Avoidance/Hospital Discharge within Cardiff and Vale UHB.In line with it’s ‘Home First’ philosophy, Cardiff and Vale UHB remain committed to providing community based services that enable patients to remain at home/return home as soon as possible after a health event.The UHB acknowledges the invaluable role that the Voluntary Sector can play is supporting individuals with low level support and wellbeing needs, where intervention by Statutory Services is not required.The key aims of the service are to:-- to provide short-term home based practical/emotional and wellbeing support for individuals referred via the wards, Emergency Unit or Cardiff Community Resource Team- To monitor client needs and assist in liaison between the individual and statutory, private and/or other voluntary agencies with the aim of regaining and retaining the patients independence within the community- As part of the interaction with clients, promote key public health messages and safety advice, that may assist the client to maintain themselves in the community- On discharge from the service, ensure clients have relevant information as to local support services that again, could assist them to maintain independence at homeNOTE: The authority is using eTenderwales to carry out this procurement process.
